Designer looking for a Programmer for 1 job

  PHP script for quiz/survey pages.
  
  I have a new job which requires a PHP programmer to create 4 to 6 quizzes/surveys. Each quiz will have a series of Yes/No questions as well as radio buttons to select. Answers will have values which add up to a final quiz score after the form is submitted. The score will determine which of 3 results pages a visitor is sent to. Submitted results will need to be emailed to the client. This email address will need to be cloaked from spambots and security measures need to be used to block unscrupulous hackers from abusing the script. Also, form may need to use a Captcha to stop auto-submission scripts.
