  Looking for a simple web uploader that our clients can use to send large files directly to our FTP.
  
  We would like to be able to send 1 link to any user containing a form in which -
  - Required Field 1 is Email Address
  - Required Field 2 is First/Last Name
  - File Input Button that allows multiple files from their computer (no file size limitations) to be selected.
  - Submit Button
  
  Then on the back end the files will be uploaded to a folder on our FTP named "USER NAME"_"USER EMAIL ADDRESS"
