  I need a PDF form created. I have illustrator files that can be converted or you can recreate it.
  
  Advanced knowledge of creating Acrobat forms is necessary.
  
  You will create a PDF form several pages long. This form needs to be fillable for online use. You need to know how to create a button that adds more fields. For example, if I want someone to list all their medications and I only create 3 fields for them but they take 4 or 5 medications, then I want to have a plus button or add button to add another field so they can type in another medication.
  
  Also, I want to be able for this form to be sent to someone via the Internet when the form is finished. Please only apply for this job if you really know how to do this kind of work. I know how to do this but I don't have time to do it. I will design the form and you can use the illustrator files I create to start it but you will have to make sure all the field names are correct etc. We also need to make sure we encrypt this on transmission.
