Programmer looking for a Designer for 1 job

  I am looking for someone to create a design for a web app I have written. The app is for use at my school (I'm a teacher) and mostly involves data entry and display. I'd like a design that appeals to both children an adults - something simple and uncluttered with nicely formatted forms and data tables.
