Programmer looking for a Designer for 1 job

  Hello, i am in need of basicly one single and simple page.
  
  I am open to suggestions on the general colours used.
  
  The current website address will be provided upon request.
  
  I would rather not have any images on the website. (Fast loading)
  I would rather xhtml and css where used. (Strict is ideal)
  
  I do not need any content added to the site (i will do that) - I just need a basic, extremely basic layout.
  
  General page layout:
  Title
  top menu bar
  side menu bar
  content (empty)
  bottom menu bar
  
  CSS is required to take into consideration that input feilds will be used, text boxes and check boxes.
  
  
  On top of this main page - i require a very small "menu" type bar which would appear on the top of another website. The size of this bar (to get a rough idea of how little work there is - would be around 4 lines - text box, checkboxes and some text).
